You can Renew your U.S. Passport if :

• You have a passport that is not damaged, and
• Issued not more than 15 years ago; and
• You were over the age of 16 when it was issued; and
• Still have the same name as in passport (or you can legally document your name change with an original Marriage Certificate, Divorce Decree or Court Order)

The requirements are as follows:

1. Your most RECENT PASSPORT .
2. Completed and Signed Application Form DS-82. Please note that only the online DS-82 application forms will be acceptable.
3. Two Passport Size Photos
   • Your photographs must be:
   • 2x2 inches in size
   • Identical
   • Taken within the past 6 months, showing current appearance
   • Color
   • Full face, front view with a plain white or off-white background
   • Between 1 inch and 1 3/8 inches from the bottom of the chin to the top of the head
   • Taken in normal street attire
   • Do not wear a hat or headgear that obscures the hair or hairline.
4. Service Letter - In order to process your request properly please print, complete and send this form together with all the documents.
5. Letter of Authorization - completed and signed.
